French Bulldogue - Burn

Dog - bite wound

Pathology category: traumatic injury 

Description: the dog is brought in urgently for bite wounds. The subject presents with about 70/80% of the skin cleaved. The wounds are more or less deep and wide with involvement of multiple planes and much loss of substance. Immediate emergency surgery is performed to clean and suture, inserting numerous drains. Double systemic antibiotic therapy, local intralesional antibiotic therapies, intravenous fluids and pain medications are prescribed. The next day the dog is alert and eating. Dressings with saline and emollient lotions/pomades are performed several times daily; local antibiotic therapy to try to minimize the inevitable risk of tissue necrosis. After 7 days, necrotic areas are surgically removed and Hypermix® medicated gauze and Hypermix®  gel formulation is introduced, removing local lotions and antibiotics from therapy. Wounds are surgically treated daily, washed with saline, and massaged several times a day with Hypermix®  gel, both on the wound and margins. Applications and massages are also done several times a day by the owners. Despite the granulation of the wounds, a third antibiotic is inserted about 2 weeks after the injury. The wounds continue to be medicated daily with the usual method: curettage, washing with saline, massage with Hypermix®  gel, Hypermix®  medicated gauze, for at least 2 months. About 2 months later, the wounds on the body are almost all healed, while the wound on the neck (the largest one, about 20x20 cm) still remains open. Complete healing occurs after about 4 months, with complete regrowth of hair.

Description: Dog, female, 5.5 years old. Bite wound with necrosis of large tissue area and cephalic vein involvement.

Mode of application: The wound was initially treated by the owner with honey, for 20 days. Subsequently, after extensive surgical curettage with removal of necrotic tissues and part of the biceps brachii, the wound was treated by application of platelet gel (PRP), prepared at the laboratory of the Department of Veterinary Medical Sciences in Parma, and HYPERMIX®, daily the first week and every other day from the second week onward, resulting in a dressing every three to four days, one month after the start of treatment. After two months, complications occurred due to poor management of the animal and venous and lymphatic stasis, and for these reasons the limb had to be amputated.

Course: started treatment February 2014, ended May 2014.

Description: During treatment, physiological functions remained normal. On the second day, it showed disappearance of the frenetic-compulsive scratching behavior, and the lesion showed marked reduction of the perilesional inflammatory collar. The area affected by dysepithelialization and erosion of the superficial dermis layer showed proggressive resolution of the inflammatory phase in favor of the granulation phase, with progressive control of exudation, within 3 days and subsequent complete re-epithelialization of the surface within 25 days after the start of Hypermix® treatment. The reappearance of hair coverage was achieved from about the third to fourth week. Daily application was always smooth due to lack of any painful reaction and was discontinued upon complete re-epithelialization of the lesion. No local or general adverse reactions occurred. No recurrences.

Male Labrador dog - Otohematoma

Description: Under general anesthesia, a pocket was incised on the inner pavilion of the ear, drained the hematoma and applied the appropriate sponge by mattress sutures. Then a protective bandage was made from the ear, adhering the convex surface of the pinna above the head. Daily morning and evening use of Hypermix® gel cream was recommended after washing with mild soap and water (Marseille). As a precaution, the patient was prescribed systemic pain relief therapy in addition to local otitis therapy based on the outcome of the ear swab. Post-operative follow-ups were performed at 3-5 day intervals, with bandage change and application of Hypermix® cream on the concave surface of the pinna. At 15 days after surgery, healing was complete and the sutures and bandage were removed.

Description: 6-year-old mascio dog. He arrived home after 3 days and had a deep wound on his right forelimb: probable trap wound. The paw presented very swollen, cold and sore. After cleaning him, we initially kept the limb in warm-warm water to reactivate circulation. Then, after surgical curettage, we began daily laser therapy with Hypermix® application and bandaging. The third photo is after curettage, laser therapy, followed by Hypermix® application. Later, after the ninth application, the client had stopped the sessions but the dog regained function and the hair grew perfectly. All at the beginning of the therapy in 28 days.

Description: presence of a wood splinter embedded in the nail plate, at the level of the left front 4th toe, with subsequent detachment of the nail. The dog manifested intense pain, with lameness and failure to support the limb and a tendency to lick itself. Description: Run over on 18/10/2014, sustaining skin lesions: a small abrasion on medial face of distal end of right hind limb + extensive lesions on outer face of left hind limb and caudal part of back, with partial detachment of skin from underlying tissues.

PROTOCOL: Initially performed surgical grooming with insertion of drains, but on 22/10 at the first checkup extensive areas of skin necrosis were already evident (hence the photos). Performed new wound grooming, following which we opted for healing by second intention, with Hypermix® dressings and connettivine gauze. Connettivine gauze was used only in the initial stages (October 22-November 4). Thereafter we switched to the use of Hypermix® gauze, and in the last few days we used Hypermix® vials and that was it. Daily dressings from 22/10 to 04/11, then every other day until 01/12, and then twice a week until 18/12. On 22/10 the CBC showed important leukocytosis with marked neutrophilia, so Clindamycin PO was prescribed at a dose of 11mg/Kg, administered until 12/11, when the CBC normalized. On Dec. 12, the wounds were all healed and perfectly cyclized.

Description: The cat was found on 18/01/2016 by two boys along a provincial road in the Ferrara area, in poor general condition, as she was cachectic and had significant respiratory difficulties, during transport they reported hearing her coughing. Objective examination: thinness, moderate dehydration (5%), significant flea infestation, large lacerated wound bruised chin and neck region (suspected bite injury), slight increase in murmur, coughing. Fiv-felv tests both of negative result, and stool and hematobiochemical tests were performed. Description: Symptomatic therapy was started with a broad-spectrum antibiotic (penicillin+streptomycin, 1 ml/10 kg p.v.) and prednisolone (15 mg IM). After a few days, two skin lesions appeared on both hind limbs, one in the lateral region of the right thigh and one at the level of the foot and left digital pad. Localized hypoperfusion was suspected. The treatment protocol, chosen according to the owner's availability, includes:

A. Trichotomy of the part.

B. Disinfection of intact skin with iodopovidone washes.

C. Wound cleansing by pressure washing (with 20 ml syringe) with Ringer Lactate.

D. Cleansing with chlorhexidine 0.05%.

E. Surgical grooming and removal of necrotic tissues.

The cat was discharged and advised to the owner to perform a compress with water and Amuchina in a 3:1 ratio, at least twice a day, and to return to the clinic for follow-up after a few days. At the next visit, therapy was then undertaken with Hypermix®, a natural product obtained from plant extracts that assists healing, has soothing, anti-inflammatory and antimicrobial effects: it creates an environment on the wound that is impenetrable to bacteria due to the presence of medium- and short-chain fatty acids in the formulation. It does not require bandaging and use of disinfectants. The advantage of this preparation is that it can be used throughout the healing process, regardless of the healing stage, and thus avoids errors derived from incorrect assessments. After one month of treatment, hypomyotrophy and poor granulation tissue formation were detected, although the wound was clean and free of contaminants: the limb allowed a supportive support that was not perfect, but sufficient to guarantee the subject some autonomy. The injury to the plantar pad was significantly improved. The bottom of the wound of the right thigh was then re-cruised and the margins sutured, continuing Hypermix® therapy on the left hind foot. After two months, the left hind limb suddenly exhibited extensive necrotic phenomena: the owner continued not to consent to collateral examinations but never considered the recurrent possibility to euthanize the subject, so amputation of the limb was proposed. In contrast, the wound on the right thigh was found to be markedly improved to complete healing after the fourth month.

Description: January 31 enterectomy for foreign body ingestion, analgesic and antibiotic therapy in the po. February 2 at the time of discharge algia at the level of the ribs. Bilateral swelling at thoracic level with subcutaneous collection at first localized then phlegmonous in character affecting the entire back of the dog following adverse reaction to subcutaneously injected drugs with bacterial infection (deep pyoderma). Feb. 23, febrile temperature (39.5), marked back pain, III/IV degree lameness at the right hindquarters, extensive skin necrosis with thick crust formation at the level of the first thoracic vertebrae and numerous fistulous paths groaning pus to the base of the tail and lateral aspect of the right thigh. Grooming and cleansing of the wound limiting at first the marked exposure of deep tissues with radical surgery. Bacteriological examination and related antibiogram (staphylococcus intermedius). Treatment protocol with cephalexin, enrofloxacin, tramodol, carprofen. Hypermix® gel was also applied to the extensive skin lesion under the crust, a bandage was applied at the trunk level by overlaying the gel with gauze medicated with Hypermix®, germane cotton wool, and a knitted onesie to keep the lesion moist and protect it from accidental damage. On the fifth day from the start of therapy, the subject's general condition definitely improved, and after seven days from the start of Hypermix® therapy, the gradual replacement of necrotic tissue with granulation tissue was observed. The lesion gradually demarcated and was groomed with each dressing. The dressings were daily in the first week, then every other day and finally every third day until after sixty days the subject was considered healed.

Description: This is a four-year-old male pinscher dog who was attacked by a large dog. After surgical courettage with drainage, the situation had improved from an infection standpoint but the stitches had not held. The wounds were multiple with lack of abundant epidermal tissue. We then started laser therapy with the application of Hypermix®. The dog underwent 11 total sessions and the frequency, due to my lack of time, was three times a week, so in 28 days the wound was completely closed. When he was not doing therapy, the dog was bandaged to avoid soiling at the site and to allow local application of Hypermix® gauze and ointment.

Description: Rat, female. Surgical wound - Subcutaneous cyst.

Product used: HYPERMIX® oily formulation

Mode of application: Surgical operation for removal of subcutaneous cyst on the right side of the back (see first photo). Over the next few days, the rat started to remove the stitches and scratch the wound, causing the suture to tear, resulting in infection. On the recommendation of the veterinarian, oral antibiotic was started, 2 times a day. The next day the wound does not improve, and each time the scab forms, the rat removes it further enlarging the wound. On the eighth day after surgery, treatment is started with  HYPERMIX® oily formulation always combined with antibiotic treatment, applying 2 drops directly to the wound twice a day. After five days, the wound shrinks and cleanses, until completely healed.

Overall, application of HYPERMIX® twice daily for 10 days. The hair grew back without leaving evidence on the treated part.
